Fees
and Payment Policy:
In
an effort to keep orthodontic fees down while maintaining
the highest level of professional care, we have established
our financial policy as follows:
- If
full payment is made at the beginning of treatment, we will
offer a fee discount.
- To
fit the needs of our patients, a financial arrangement can
be made to extend your payments over a period of months
following an appropriate down payment.
- For
your convenience, we offer Visa, MasterCard, Discover, and
American Express service.
Insurance:
If
you have insurance we will help you to determine the coverage
you have available. We ask that you assign your insurance
benefit to us. The balance will be arranged for you to pay
as listed above.
Professional
care is provided to you, our patient, and not to an insurance
company. Thus, the insurance company is responsible to you,
the patient. We will help you in every way we can in filing
your claim and handling insurance questions from our office
on your behalf.
Emergency
Care:
An
orthodontic emergency is when the patient is experiencing
discomfort by a sticking wire or a broken appliance. We reserve
a limited time each day so that emergency patients may be
scheduled quickly to make them comfortable. Replacement of
loose bands and brackets will be rescheduled for an appointment
time that allows for a longer procedure.

For
your convenience will have included several PDF forms you
can download, print, and fill out before you come to the office.
When completed, please fax these forms to us at 512-732-2601.
This will save you time during your office visit. If you don't
have access to a fax machine, you can bring the completed
forms with your to your appointment. Click on the links below
